Library Annex Request Form

Library staff will retrieve Library Annex items daily Monday through Friday. Items requested by 2:00 PM will be available by 4:00 PM the same day. Once retrieved, items will be held for the requester for ten days at the Morris Library Circulation Desk or at a Branch Library if so indicated. The same borrowing policies will apply to Library Annex items as applied to those items when they were located in their originating library.

This form, called a CAPTCHA, ensures that a real person, not a computer, is submitting the online form. The CAPTCHA serves as a security measure, minimizing spam for the Library's staff.
