INSTRUCTIONAL MEDIA COLLECTION FORMS

Web Form Submission (Submit online) or Media Scheduling Request (PDF for campus mail, drop-off, or fax)

Use this form to request media items be sent to a designated campus delivery site for future use in classrooms and lecture halls, or for pick-up at the Instructional Media Collection department service desk.

Deliveries of scheduled videos to designated UD campus delivery sites will end January 4, 2010. Beginning with the Winter term 2010 all films and videos scheduled in advance for teaching and research must be picked up in person at the Morris Library Instructional Media Collection Department. Videos scheduled for use at UD campuses in Wilmington and downstate will continue to be delivered to those locations. For further information, click here.


Please note: Scheduling requests for videos need to be received by the Instructional Media Collection Department at least seven days prior to the first scheduled use or show date. Materials needed within seven days of the use date can be checked out at the Instructional Media Collection department service desk on a walk in basis without scheduling.

Authorized Borrower Card Application for Faculty

Faculty may designate a research assistant or staff to circulate media materials at the Instructional Media service desk on his or her behalf if the designee has an Authorized Borrower Card in the faculty's name. After this form has been completed and signed by the faculty member, it can be faxed to (302) 831-4153 or mailed to the Morris Library Circulation Desk. Please allow five working days for processing. To protect faculty members from having Authorized Borrower Card forms submitted in their name without their knowledge, processed Authorized Borrower Cards are mailed to the faculty member for distribution and are not given directly to the authorized representative.

Student Request to Schedule Media (PDF for campus mail, drop-off, or fax)

Use this form to request a currently enrolled UD undergraduate student be able to schedule restricted media items under a faculty member's name. The form must be submitted between 8am and 5pm, Monday through Friday.

Request for Nonprint Materials (PDF for campus mail, drop-off, or fax)

Use this form to request items for purchase for addition to the Library's Instructional Media Collection.

Web Form Submission (Online) or Viewing Room Scheduling (PDF for campus mail, drop-off, or fax)

Use either form to schedule the Media Viewing Room at least seven days prior to the intended use date for use with a class. Within seven days, please call 831-1042 during office hours (8am-5pm, Monday - Friday).

Please Note: Scheduling requests of media items for use in the Viewing Room must be submitted via a Viewing Room Scheduling or Web Form Submission. To inquire about the availability of the Viewing Room for a specific date and time prior to submitting a request, call 831-1042 during office hours.
